I note that drugs played a in that crash, also of note were the comments about booking arrangements and lack of any supervision, something of an irony considering it's practically standard practice now. Before retirement I doubt if I spoke to anyone when booking correctly on about 60% of occasions, booking on by automated phone system was the standard procedure(probably still is).
