You accidentally clicked on it when you clicked in the reply box to begin typing. It is easy to do if you fumble the mouse -- that's why we occasionally see posts in all bold or all italic. To avoid the problem, when clicking in the reply box, always click near the bottom of it. This is typical poor programming from IPS, there is no reason why the toolbar shouldn't be always visible.
