I've been looking for this book for ages but to no avail. It is a longshot, but hopefully someone may have a unwanted copy kicking around somewhere! They were so helpful in providing assistance and I couldn't recommend them more highly. They also have a range of other interesting titles available so I will be back for a couple of other publications in due course.
