Reversing trains

The SWR services to Yeovil Pen Mill have to reverse en route, either at Yeovil Junction or Westbury, depending which route they've taken to get there. Likewise the Summer Saturday service to Corfe Castle reverses at Yeovil Junction, Yeovil Pen Mill, Weymouth and Wareham. And then all those places again on the way back.

And Paddington & Euston trains left in opposite directions

Same at Exeter St Davids where GWR to Paddington went North & SR to Waterloo went South.

And Plymouth North Road - GWR to London to the east and LSWR to London to the west.

Can only enter via Proof House, it's possible it exits the same way and goes via Aston and Bescot to Wolves.

Some XC trains still reverse in Birmingham to get to/from the NE

I don't know the current situation in practice, but there are four possible routes through New Street for a Bristol / (Manchester / Liverpool / Glasgow) train, two of which involve reversal. Heading north, arrive either via the Camp Hill line or the West Suburban, depart via the Grand Junction line through Aston or the Stour Valley line. There are further possibilities for a such a train avoiding New Street - the Summer Saturday Pines Express ran via Walsall, Water Orton, and the St Andrews curve.
